The easiest way to become a patient at Cricketfield Surgery is by registering online. It doesn’t take long, and it gets processed quickly.

We welcome new patients who live within our GP surgery area. Our area map is displayed at the bottom of this page. When you register you will be allocated a named GP who will be responsible for the coordination of your care.  However, we have a variety of clinicians and patient support team members who can assist with your medical and wellbeing needs. We aim to get you the right person in the right time for your specific needs.

New patients are asked to complete a thorough questionnaire to ensure we have a good medical history for you.  If you are on any regular medication, you may be offered an appointment to discuss this with a clinician. You can also ask for help with any particular medical, social or psychological difficulty.

Your medical records from your previous GP surgery will be requested and transferred to us but this can take several weeks. However, medical treatment is available from about 1 week after we receive an online registration form. You can opt to come into the practice and complete a hardcopy registration form, but this can take 2 to 3 weeks to process and for medical treatment to be available to you. If you have a prescription that needs renewing in this time you can still request this from your previous GP surgery regardless of location and they can arrange for the prescription to go to a local pharmacy.

Please note it is preferable for us to see ID when you register with us, but it is not essential, unless you want online access. You can download the NHS App to gain online access as it is quick and easy to photograph your ID for the system to read and approve. If you want us to set you up with online access you will have to complete the additional form below and provide ID to ensure we are giving access to your confidential medical records appropriately. If you would like someone else to help you manage your healthcare, you should request they have third party access otherwise we cannot share any information with them or allow them to assist you.

You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.

Online Access

You are able to access your records online to order repeat prescriptions and to make and cancel appointments. To obtain a use ID and password to access these facilities you first need to complete an application form. This form should be returned in person to the Surgery together with photographic ID – e.g. passport or driving licence.

You will then be issued with a password granting access to the services available.

If you do not have access to a printer you can chose to email the completed form to us at cricketfield.admin@nhs.net, then pop along with the above ID in person to the surgery.
