Wellbeing Team

Our Wellbeing Team supports patients with non-medical and lifestyle needs that affect health and wellbeing. We work closely with the clinical team to offer practical, personalised support alongside your GP care.

Our service is fully person-centred, meaning we take the time to understand what matters to you and tailor our support around your individual needs. We offer a friendly meet-and-greet welcome and, where helpful, personal introductions to members of our team and other professionals involved in your care.

We are a vulnerable-aware and disability-aware service and are committed to making any visit as comfortable and accessible as possible. Reasonable adjustments can be made to support physical, sensory, learning, or emotional needs, and we encourage you to let us know how we can best support you.

Social Prescribing & Community Support

(Social Prescriber, Health & Wellbeing coach, Veterans lead)

Nadine can help if things like stress, loneliness, caring responsibilities, finances, housing or life changes are impacting your wellbeing.

Support may include:

  • One-to-one conversations

  • Connecting you with local groups, activities and services

  • Help with forms, benefits or signposting

Debbie

Dietetic Support (Dietitian)

Debbie provides tailored dietary advice for adults, children and families, including support with:

  • Weight loss or weight gain

  • Weight loss due to illness, eating difficulties or older age

  • IBS and digestive problems

  • Food allergies and intolerances

  • Diet and menopause

  • Long-term conditions such as diabetes, high blood pressure, cardiovascular disease and COPD

Advice is realistic, supportive and designed to fit everyday life.
