Appointments from a multi-disciplinary team to ensure you get the right person at the right time for:

  • Your new severe symptoms for physical or mental health matters
  • Your prolonged symptoms that you haven’t been able to get sufficient help with through self-care, online NHS recommended advice, or a Pharmacy
  • Your long-term health conditions
  • Your wider wellbeing if you have any social, relationship, carers, parenting, financial or employment issues affecting your health
