JOB VACANCY – Clinical Pharmacist

Templer Care Network is looking to recruit an innovative and enthusiastic pharmacist who wants to work within primary care.
The practices within the network have a long history of collaborative working, are within close proximity and between them look after 50,000 patients.
We are seeking to recruit a clinical pharmacist who is self-motivated and keen to learn. You must be able to follow legal, ethical, professional and organisational policies/procedures. Involve patients in decisions about prescribed medicines and support adherence as per NICE guidelines. The role is varied, clinical and management support will be provided.
In this role you will act as part of a multi-professional NHS team in providing clinical pharmacy services that will protect, maintain and restore the health of patients.
The practices have been rated Good or Outstanding by CQC, with consistent, positive GP patient survey results which demonstrate the strong patient centred customer focus and efficiency of our teams.
Some of the areas the candidate would be asked to focus their attention on would be:
Responsibilities include (But are not limited to):
- Medication Queries
- Medicines Reconciliation
- Face to Face and Telephone Medication Reviews and Callbacks
- Management of Chronic Diseases
- Support and Develop safe prescribing procedures
- Work alongside Nurses, GP’s and other HCP’s
- Reviewing QOF to identify Patient’s in need of intervention
- Attend Practice and Network meetings
- Quality Improvement projects
Preferred but not essential:
- Clinical Diploma / Practice Pharmacist Experience
- Prescriber OR Currently on the NMP course
Job Types: Full-time, Part-time
The practices all use the same clinical system – TPP SystmOne
Salary dependent on skill set, experience and qualifications.
